Agreement to Term

Travalab provides professional mobile phlebotomy services, bringing convenient blood drawingservices directly to your location. Our services include specimen collection, handling, and transportation to laboratories, along with scheduling and coordination of these services through our digital platform.

Eligibility and Responsibilities

To use our services, you must be at least 18 years old or have parent/guardian consent. You are responsible for providing accurate information and maintaining a safe environment for service delivery. We rely on you to inform us of relevant medical conditions and follow preparation instructions for blood draws.

Scheduling and Cancellation

Our appointment system is designed to provide convenient, reliable service. Appointments must be scheduled through our website or app and require confirmation from our phlebotomists. Werequire 24-hour notice for cancellations to maintain service quality for all patients. Late cancellations or no-shows may incur fees.

Payment Terms

Payment is required at the time of service, and we accept major credit cards and other approved payment methods. Our pricing includes:

• Base service fee
• Additional fees for travel beyond service areas
• Cancellation fees when applicable

Insurance claims are the responsibility of the patient, though we provide necessary documentation for submission.

Medical Consent and Authorization

Using our services indicates your consent to blood draw procedures and authorization forsample testing as ordered. You acknowledge the inherent risks of blood draws and agree to follow all preparation instructions provided by our team.

Dispute Resolution and Arbitration

Any disputes between you and Travalab will be resolved through binding, individual arbitration,conducted by the American Arbitration Association under their Consumer Rules. You may optout of arbitration by emailing support@travalab.com within 30 days of accepting these terms.

Service Limitations

We strive to provide reliable service but may be limited by:

• Geographic service area boundaries

• Force majeure events
